工作、課業忙碌到連尿尿的時間都要省嗎？當心憋到尿不出來！醫師表示，憋尿、久坐的習慣，會讓提肛肌長時間處於壓迫緊繃狀態，久了肌肉忘記如何放鬆，導致排尿困難的窘境。

39歲王小姐，經常因工作憋尿久坐，最近幾個月自覺排尿不順暢，就醫檢查發現，餘尿高達300cc。還有一名52歲女性，有急尿、頻尿的困擾，但每次上廁所都要等30到60秒以上才能尿出，且經常感覺沒尿乾淨，有時一咳嗽就忍不住尿出來，檢查發現餘尿達150cc。
替上述患者診治的書田診所泌尿科主任醫師杜元博表示，婦女排尿困難多是因膀胱出口阻塞，骨盆底肌肉協調困難。常見的誘發因素包含：過動性膀胱發生、骨盆底肌肉放鬆不能、尿道外括約肌痙攣、膀胱頸功能失調、膀胱出口外來壓迫、尿道狹窄、子宮脫垂、慢性膀胱發炎、免疫神經疾病等。
杜元博提到，最近一些研究指出，婦女膀胱出口排尿阻塞的機率約從2.7％到25％都有報告。一般以膀胱刺激症狀比膀胱排空的症狀來的更為明顯或嚴重，膀胱刺激症狀是指如急尿、頻尿、尿失禁等；膀胱排空症狀如排尿遲緩、尿流細小、尿流中斷及殘尿感等。
此外，習慣憋尿者，也可能發生憋到尿不出來的情形。杜元博說，憋尿、久坐會使提肛肌長時間處於壓迫緊繃狀態，久了肌肉忘記如何放鬆，協調失能，導致排尿困難尿不出來。目前治療方式包括藥物（括約肌肌肉鬆弛劑、血液循環藥物、抗攝護腺肥大藥物，女性荷爾蒙等）、注射肉毒桿菌、手術、凱格爾氏運動、磁波椅復健治療等。凱格爾氏運動須每天早中晚各做20次，1天共60次，至少執行3個月以上才會有明顯效果。
磁波椅復健治療是透過椅子下方的線圈，產出7到14公分電流磁場刺激骨盆肌肉，使肌肉反覆收縮和鬆弛，回復原有彈性與功能，每週2到3次，每次療程20到40分鐘，連續3週，是非侵入性、非接觸性的治療方式，但須自費，但有嚴重骨盆腔感染者不適用。
最後杜元博提醒，中老年女性常有排尿困難或尿失禁困擾，往往對生活造成影響，且長期排尿困難易引起泌尿道感染，甚至造成膀胱、腎臟功能受損、尿毒症，不可不慎。目前治療方式有多種，可與醫師討論選擇適合的方式，以利改善生活品質，同時避免併發症的產生。
