僵直性脊椎炎不只有下背痛 醫：這些症狀易忽略
年紀輕輕就腰痠背痛、整天提不起勁，甚至嚴重駝背，看起來無精打采，可別以為只是疲勞、姿勢不良所造成，小心恐怕是僵直性脊椎炎惹的禍！
歌手周杰倫就是知名病友，還曾透露，「僵直性脊椎炎發作起來比經痛還要痛！」甚至在發病期間，得讓媽媽蹲下替他穿襪子，周杰倫也曾感嘆，「如果以後我的小孩說『爸爸抱我、爸爸揹我』，我要怎麼回答『對不起我抱不動你』。」讓周杰倫心裡很難受。
其實不少藝人都飽受僵直性脊椎炎之苦，像是藝人阿Ken、李易和余天等都是病友。
僵直性脊椎炎(ankylosing spondylitis，簡稱AS)是一種慢性自體免疫疾病，因為自體免疫系統受到破壞，導致脊椎、關節等處出現慢性發炎現象，好發於20?40歲男性，是女性病患的1.5倍，台灣每1萬人中就有33.7人罹患，盛行率居亞洲之首。患者因長期慢性發炎導致脊椎韌帶、關節鈣化，脊椎沾黏變形，嚴重時可能因此駝背、抬不起頭，甚至是無法行走。
初期症狀易輕忽 醫：常以為只是姿勢不良
是醫師也是病友、中華民國風溼疾病醫學會理事長蔡文展表示，僵直性脊椎炎好發於年輕男性，常因為疾病意識感較低，
發病初期誤以為是運動傷害、姿勢不良或過度疲勞，而延誤診斷、錯失黃金治療時機，導致脊椎變形、駝背等不可逆的傷害。
僵直性脊椎炎通常會有明顯的家族病史，但除了遺傳因素之外，下列初期症狀往往容易被忽略、誤判，民眾得多加留意：
●
腰痠背痛：症狀持續三個月以上，尤其是下背部有僵硬感、疼痛，在運動後較舒緩，休息後反而感覺較痛。
●
晨間僵硬：早上起床下背特別疼痛，甚至身體僵硬起不了床，持續30分鐘至一小時。
● 莫名疲累：蔡文展表示，就像長期處於「重感冒般」的疲累感，補眠也無法緩解，嚴重時甚至整個人提不起精神、充滿無力感，恐怕會影響職場上的觀感。
●
虹彩炎：中華民國風濕病醫學會與僵直性脊椎炎關懷協會指出，台灣約有三至四成僵直性脊椎炎患者會併發虹彩炎，臨床症狀會出現眼睛紅腫充血、視力模糊等，若延誤治療，恐造成瞳孔變形、青光眼等問題。

著骨點發炎：
有些病患會出現腳後跟的阿基里斯腱及足底筋膜疼痛，蔡文展表示，
不少病患容易被誤診為單純的足底筋膜炎，但一般年輕人其實很少會有足底筋膜炎，所以要觀察是否有其他症狀才能確診，通常也會伴隨身上其他肌腱發炎，像是出現長期胸痛的狀況，就是肋間肌著骨點發炎。

關節發炎：僵直性脊椎炎患者除了脊椎病變外，也可能會有周邊關節炎，常侵犯下肢關節，如髖關節，膝關節及踝關節。
不動才會痛 醫生：游泳是最好的運動
（游泳能降低脊椎壓力，是相當適合僵直性脊椎患者的運動。圖片來源：shutterstock）一般疼痛通常會「越動越痛」，但僵直性脊椎炎卻是「不動才會痛」，因此蔡文展鼓勵患者多運動，「
對僵直性脊椎炎患者來說，游泳是最好的運動，」因為能藉由水的浮力，讓脊椎不必承受自身體重，能夠真正放鬆且不容易受傷，另外患者初期也可以騎腳踏車，同樣能降低脊椎壓力，「但
不建議脊椎已經沾黏的病患跑步，因為會有跳躍動作，脊椎承受更大壓力，若是跑步跌倒，更可能導致骨折。」
蔡文展指出，很多病人會尋求很多偏方，甚至有病人一次就吃一整瓶黑藥丸，其實裡面主要成份是類固醇，但
針對僵直性脊椎炎主要以非類固醇消炎止痛藥和生物製劑較有療效，傳統的消炎止痛藥對五、六成病人有療效，剩下四、五成病人可能就要使用生物製劑，他也強調，「
除了藥物治療之外，病人一定要適度運動，才能避免脊椎沾黏、降低發炎現象。」
醫師也呼籲患者不必太過灰心，「其實只要及時診斷、遵循醫囑、規律用藥，要妥善控制僵直性脊椎炎並不難，」依舊有機會擁有良好的生活品質。
