別小看腸躁症問題

【華人健康網圖文提供／三采文化】功能性腸胃疾病依照症狀位置分為「上、下」功能性腸胃疾病是指一群會反覆發作，卻無法以解剖、結構性病變和生化檢查異常來解釋的胃腸症。上─喉嚨有異物感、胸悶、胃食道逆流。下─大腸激躁症。這類患者的症狀可能複合式地存在，例如：有些人有胃食道逆流，又合併大腸激躁症，有人焦慮不安，同時又有頻尿（間質性膀胱炎）症狀，但仍會以一個最主要而明顯的症狀（可能是腹瀉、便祕，或是頻尿等）來呈現。
近年來，隨著醫界對黏膜免疫學和腸道菌叢的了解更多，雖然大腸激躁症患者的內視鏡檢查狀似正常，但在上皮細胞切片的組織學上，即使沒有白血球浸潤，但是有一大部分的病人腸道黏膜的T淋巴球和肥大細胞數目增加，同時，細胞激素、組織胺、蛋白酶等濃度增高，其中的蛋白酶會破壞上皮細胞的緊密連接，使滲透度增加，而滲透度改變會使腸腔內的抗原（如食物、細菌）進入黏膜固有層，使神經末梢與肌肉細胞受到了干擾！
2006年，美國腸胃學期刊中登錄普醫師（Dr.Dunlop）的研究論文，指出有些腹瀉型腸躁症患者雖然並未發生過腸道感染，但小腸前段的滲透度是增加的。這代表感染只是腸躁症的機轉之一，很有可能的是，感染只是讓患者長久以來腸子滲透度異常的問題凸顯、浮出了檯面，而且以前的研究都只強調迴腸末端和大腸，而忽略了腸道的整體性。
所以綜合來說，大腸激躁症跟腸道滲透度、腦部、週邊神經、體內的輕微發炎有關。當某人具有弱基因（家族中有腸躁症的患者），而且恰好遇到感染、使用了抗生素、或是承受了慢性壓力，就有可能誘發腸躁症。另外要注意的是，這類病人同時被發現有過敏體質，常伴隨著慢性濕疹、氣喘和花粉熱等這類系統性免疫疾病。
半數的腸躁症患者合併有腸漏症根據排便習慣，大腸激躁症可分為「腹瀉型」與「便祕型」、「輪替型」三大類。不管是腹瀉型、便祕型、輪替型，這三大類的腸躁症都可能跟腸道中黏著小帶（zo1）的濃度改變有關，這時腸子的滲透度也跟著變了，用小腸滲透力分析（單糖/雙糖）的方式來檢驗，會發現有12～50％的異常比例，而腹瀉型的腸躁症與腸漏的關聯性更高；合併有感染症狀的患者，異常比例也是最高。
簡單來說，約有半數的腸躁症患者合併有腸漏症但是反推回來，不是所有的腸漏症患者都會產生大腸激躁症，會扣下罹病板機的因子還包括基因、壓力、環境、感染、嬰兒期創傷等，需要許多條件的配合才會產生疾病。症狀的重疊性但不管是什麼型，大腸激躁症患者通常會出現失眠、緊張、頭痛、疲勞、非心因性胸痛、心口灼熱、吞嚥困難等合併症狀，甚至會有疼痛膀胱症後群、間質性膀胱炎、顳顎關節症候群等。情形時好時壞，通常在情緒差、壓力大時，症狀就會惡化，而身體病痛又會影響心情，如此惡性循環使患者身心俱疲。
