乳癌患者就診時已乳房潰爛 荷爾蒙治療延命多年
80歲的陳阿嬤4年前發現罹患乳癌，面對疾病來襲，她一度驚慌、無法接受，更害怕自己撐不過化療的痛苦與副作用，幾乎放棄。所幸醫療團隊評估阿嬤身體狀況，考量其年紀及體力等因素、並不建議使用化療，與病患、家屬討論後，改採荷爾蒙療法，順利延長阿嬤存活時間，現在的她還能照顧生病住院的媳婦。
根據衛生福利部統計，癌症高居台灣十大死因之首，乳癌更位居女性癌症發生率榜首。中山醫學大學附設醫院乳房腫瘤專科醫師葉名焮主任指出，全台每年罹患乳癌的新增人數約有1萬5千人，且每18至19名婦女中就有一人可能罹患乳癌，其好發年齡層為50歲上下。早期乳癌治療方式以全切除為主，近幾年陸續引進局部切除及腫瘤整形手術、微創手術等再加上化療、荷爾蒙治療及標靶治療等，可提升乳癌患者5年存活率達近9成以上，且治療後外觀與生活幾乎不受影響。
葉名焮表示，門診上遇到不少病患努力了大半輩子、正準備享受人生，竟面臨罹癌噩耗，讓許多婦女不願正視並接受治療，葉名焮曾遇過一名40多歲患者，摸到乳房硬塊時不願就醫，選擇相信偏方治療，1年後癌症進展到第四期，已非開刀可解決，甚至有醫護人員也因害怕乳癌而怯於治療，令他十分扼腕。
在各種晚期乳癌治療方式中，葉名焮說，化療是其中一種方式,但是化療可能引發掉髮、胃口差及白血球數下降、發燒等多種副作用，相較之下荷爾蒙療法的副作用較為輕微，病患的接受度相對也比較高, 並可幫助患者維持生活品質並且達到控制疾病進展的目的。也因此，荷爾蒙療法近幾年備受重視，葉名焮就曾在門診時，遇到一名比丘尼就醫，診查時發現其乳房嚴重潰爛化膿，經施以荷爾蒙治療，延長其壽命達10多年。
其實，荷爾蒙療法使用在第一至四線的藥物種類繁多，荷爾蒙受體陽性、停經後晚期乳癌患者，使用的第一線抗荷爾蒙藥物有選擇性雌激素受體調節劑、芳香環酶抑制劑及較新的針劑荷爾蒙藥物，葉名焮指出，以上述陳阿嬤為例，原本口服荷爾蒙藥物，但因藥物副作用導致身體僵硬如機器人，在醫師建議下改使用針劑荷爾蒙藥物，現在已可正常運動、正常生活。
「乳癌第四期不叫末期！」葉名焮強調，雖然早期發現早期治療，但乳癌第四期患者也不必憂心喪志，因為隨著科技日新月異的新治療方式和藥物的出現，已大幅延長患者存活時間，乳癌已非不治之症，唯有與醫療團隊良好配合，晚期乳癌患者仍可與疾病長久共存。
