7旬翁腰酸背痛腎功能變差 竟是多發性骨髓瘤惹禍
台中75歲吳姓老翁去年因腎功能不佳，每月定期至門診追蹤， 但上個月背部開始感到劇烈疼痛，原以為是腰背部扭傷， 先至復健科求診但效果有限，不料腎功能又持續變差， 並出現食慾不振、頭昏疲倦等症狀， 轉至血液腫瘤科安排骨髓穿刺檢查，確診為多發性骨髓瘤第三期， 正透過標靶治療延緩疾病惡化。
亞洲大學附屬醫院癌症中心副院長暨血液腫瘤科主任黃文豊表示，「 多發性骨髓瘤」是血液性癌症的一種，根據統計， 台灣骨髓瘤發生率在過去25年中約上升三倍左右，致病原因主要是 體內漿細胞（白血球的一種）出現病變，產生過量同質性抗體， 取代能正常對抗外來病原的漿細胞，造成患者免疫力下降， 容易發生感染，加上這些多餘的抗體會沉積在腎臟， 患者多會出現腎功能衰退的現象。
黃文豊指出，多發性骨髓瘤好發於骨髓空腔內， 當漿細胞瘤蔓延到各處骨頭，使得骨頭被侵蝕， 患者就會出現腰痠背痛的症狀，尤其發生於前胸及背部， 因此症狀與老化的特徵相似，常見在骨科或復健科治療無效或轉成骨 折就醫才被發現，以致患者容易輕忽，通常已經錯過治療黃金時間。
黃文豊介紹，多發性骨髓瘤好發於老年人，男性多於女性， 在台灣並非常見的癌症疾病，此疾病早期通常不會有任何症狀， 唯有當癌細胞向骨髓外延展時，就會出現腰痠背痛、反覆感染、 體重減輕，甚至噁心、嘔吐、便祕、排尿困難、四肢麻木等情形。
但黃文豊強調，隨著醫療技術進步， 治療計畫會依癌症的期別和病人的症狀量身訂作， 透過全身性的化學治療和標靶治療對抗癌細胞，此外， 有時也會透過放射線局部治療阻止癌細胞生長， 但因此病的併發症容易出現在身體任何一個部位， 故需要支持性療法緩解不適，以減輕病患的痛楚及提高生活品質。
黃文豊提醒，因目前尚未得知多發性骨髓瘤的確切成因， 故應避免長期暴露於輻射線或化學物質，家中長輩若出現貧血、 下背痛、尿液中有泡泡等現象，應盡早赴醫檢查， 才能把握治療黃金期。
