親子共眠促安全感？當心窒息風險
（優活健康網記者林奐妤／編輯整理）過去二十多年來，越來越多的人主張，多數睡眠不足家長所犯的錯誤，不是沒有充分訓練寶寶獨自入睡，而是一開始就試圖訓練寶寶獨自入睡。他們指出，一直到最近，人類都是群體共眠。我們為什麼認為這些生理的必然性會突然改變？
共眠很有道理─寶寶有安全感，母親更換尿布或餵奶也不必大費周章（大多數共眠法的倡導者也是哺餵母乳的支持者）。許多國家的孩子在青春期之前都睡在父母的臥室裡，例如日本、伊拉克和印度。所以，有何不可？
親子共眠法（Co-Sleeping）技術上來說，共眠法是執行親子同室，通常很靠近彼此，常見的狀況是附加一個邊床─就像一張嬰兒床，但是可以拆掉單邊，讓母親拉到床鋪旁邊，方便半夜餵奶。問題是，大多數人談論共眠法的時候，他們實際上是在討論「共床」。在美國，共床的支持者忽略了一個非常重要且嚴重的細節：父母睡的大床。在這些普遍共床、而嬰兒猝死症比例低的國家，人們睡在薄而堅硬的床墊上、甚至是地板上，蓋著薄毯與扁枕。
然而，西方國家的人們睡著蓬鬆的枕頭、巨大柔軟的厚棉床墊、床墊幾乎像棉花層如雲朵輕軟，再配上多條床單與毯子。因此，比起獨自睡在嬰兒床的寶寶，與母親共床的寶寶死亡率高出五倍─無論是死於嬰兒猝死症、窒息或過熱。就算家中換了比較堅硬的床墊，也不會有太大的差異，因為問題不僅是床墊，還有我們生活方式，也完全都不一樣。
共床根本不是一個合理的選擇，而且研究清楚指明：不值得冒這個風險。一份匯總五項研究成果的報告，涵蓋英國將近一千五百名嬰兒猝死症個案，結論指出：如果不與雙親共床，有八成八的嬰兒原本不會死亡。美國研究人員調查八年內超過八千個不明原因的嬰兒死亡個案，在《兒科期刊》（Pediatrics）上發表，未滿四個月大的寶寶有七成四的死因，也是起因於共床。
除了少數例外，多數共床家庭的結局都不好。人們會不擇手段獲取需要的睡眠─如果寶寶半夜醒來實在令人難以忍受（尤其母親必須回到工作崗位、恢復職業世界的節奏與步調），她會將寶寶抱上床。短暫時間之內，共床通常會奏效，大家都睡得比先前好。但是─不論是誰發明了「像寶寶般安睡」這句詞彙來描述一個人睡得很好、安靜與平靜，他絕對沒有與寶寶一起共眠過。
寶寶會扭動；他們會踢腿；睡眠中，他們會嗚咽與哭泣。隨著他們長大，四肢會更長，踢人更痛，手肘、膝蓋攻擊更痛。孩子睡得很好；家長則不然。但是，當然幾天後就養成習慣了。孩子喜歡這個新的安排，任何讓他回到嬰兒床上睡覺的企圖，都很可能會遭遇到猛烈而響亮、不願入睡的抗議。在這種情況下，多數家長容易讓孩子留在大床上，決心暫時忍耐度過這段時期，等寶寶狀況好些再來處理─而，對多數家庭而言，「暫時」的結果就是永無休止。
當然，有些母親熱愛共床，特別是哺餵超過好幾個月的母親。她們享受與寶寶的親密時光；任何一位超過一、兩天沒能連續睡上三個小時的人都能證實，任何睡眠（就算是稍微被干擾的睡眠）也比沒睡覺好。但是，我數不清在診間遇過多少人，他們多年來與孩子共床，懇求我鼓勵他們的妻子或伴侶，讓孩子睡在兒童床上。
長時間的共眠（不管何種形式），就算沒有破壞他們的睡眠，也可能對伴侶的親密感產生破壞性的影響。而這些對夫妻關係的影響更負面而長遠，比幾夜難熬的睡眠訓練更棘手多了。哭泣式睡眠法可能揪心、內疚；共床則有風險、有破壞性。然而，我執業多年來與新手家長分享緩和的睡眠訓練法，卻能在幾週內讓全家睡得安全舒適。
