顯微重建叢神經損傷 恢復臂力不是夢
【健康醫療網／記者林怡亭報導】由於部分臂叢神經損傷無法自行恢復，因此及早正確的診斷評估非常重要，包括透過理學神經學檢查評估肩部上肢手部的肌肉力量和感覺異常的分布情形，以及精密的影像學檢查(如磁振造影檢查)來幫助定位臂叢神經損傷病灶。
右上肢癱軟無力 重建顯微手術恢復37歲陳女士，因車禍傷及右側頸肩，導致右側鎖骨肋骨骨折合併氣血胸，在外院緊急手術後入住加護病房；之後病況雖趨於穩定，但隨即發現右上肢癱軟無力情況未見改善，於是在出院後隨即前來手外科門診評估，並透過定期神經肌電圖檢查確診為「急性右側臂叢神經損傷」，且無短期恢復的跡象。
與手外科醫療團隊討論手術計畫後，陳女士接受了神經轉位重建顯微手術。在隨後門診追蹤及復健衛教之照料下，陳女士的右上肢功能已逐漸回復，並順利回到職場。
神經損傷上臂失能 臂叢神經在作祟手外科王榮磻主任表示，臂叢神經為一組自頸部脊髓至上肢手臂的神經叢，這些神經會支配肩部、手肘、手腕、手部的肌肉，並提供上臂表皮的感覺功能。有些臂叢神經損傷較輕微，可在數週後完全康復。然而有些臂叢神經損傷會嚴重到上臂完全失能。
造成臂叢神經損傷的原因很多，其中尤以牽引性拉傷為最多，大部份病人為年輕人騎機車車禍而造成；通常都因為車禍發生時頭頸部被強力扯離肩部，使得臂叢神經自頸椎脊髓撕裂損傷，進而導致患側癱瘓無力。由於台灣交通人口之中騎乘機車的人口比例佔有相當數量，臂叢神經因車禍而受傷的情況仍十分常見。
顯微重建治療 恢復快也治療佳針對成人臂叢神經損傷的顯微重建治療，王榮磻主任指出，近年來由於對臂叢神經的解剖位置認知、影像醫學技術革新、神經接合材料進步，使得臂叢神經損傷的手術治療有了長足之轉變。
手外科王榮磻主任表示，目前急性臂叢神經損傷之手術治療趨勢是神經轉位手術，相較於傳統神經移植
過年送禮推薦團購人氣商品修補手術，有更短的住院天數（轉位手術術後僅需2~3天觀察，移植修補手術術後往往需一週觀察），及更少的活動限制（轉位手術術後，僅需手臂吊帶保護和避免過肩姿勢，移植修補手術則需要肩頸部活動限制，避免移植處拉扯）、術後恢復時間也較快。
透過神經傳導檢查 可以看出神經變化手外科王榮磻主任指出，神經傳導檢查/肌電圖檢查是手外科醫師確診臂叢神經損傷的最佳利器；通常會在臂叢神經損傷後6週，進行第一次的神經傳導檢查/肌電圖檢查，之後在臂叢神經損傷後2～3個月，進行第二次的神經傳導檢查/肌電圖檢查，藉此觀察比較臂叢神經損傷的恢復變化。
